Library Card & Check Out Rules/Regulations
Youth Library Card Application
All card holders will accept full responsibility for all material selected and borrowed on their account, including any lost, damage, fines or fees incurred and will abide by all Library regulations.
-There will be a daily fine for all materials not returned by the due date.
-Multimedia items, including videos, DVDs, films and CD's are to be used as intended, the Library assumes no responsibility for equipment damaged that may result from use.
-Any change in contact information needs to be reported to the library.
-Using another persons library card, including a family member, is not permitted.
-Library Materials not returned after 2 months can be regarded as theft per Iowa Code 714.5 and will be turned over to the library board and police.
Applications may be filled out at the Woodbine Carnegie Public Library, or click the file below to download and print the application, then submit it to the Library.
Applicants under the age of 9 require parent/guardian signature, applicants age 9 & up may fill out application themselves.
Along with this form, please present a valid driver's license or other acceptable ID with current address and signature. This may include; state department license/photo ID, valid school ID, along with a piece of mail post marked within the last 30 days; i.e. utility bill or lease combined with a signature/photo ID.

Youth Check-out RulesBooks - Up to 10 books per child at one time may be checked out.
Magazines - Up to 3 magazines per child may be checked out at a time. Movies - Up to 2 movies per family may be checked out. A total of 10 combined items may be checked out at a time. All books, magazines, and audio books are checked out for two weeks. Movies are checked out for 3 days. |
Fines and FeesLost Materials: patrons could be charged the replacement price, the fine, and the cost of a restricted certified letter.
Damaged Materials: charges for damage may vary with the extent of the damage. Returned Check Fee: a fee will be assessed for checks returned unpaid. This fee will be the same as that assessed by the City of Woodbine for returned checks. |
